We can not guarantee coverage until we verify eligibility of the individual. Call our office we would be happy to check.

For our office to verify your coverage,  we will need the following:

1. Your phone number so that we can contact you
2. Group, plan or policy number
3. Social security number or ID of the insured
4. Member and patient’s date of birth
5. Name of employer
6. Would you like to set up an appointment? Your family members may be covered by your insurance too and set up an appointment for them as well.
